Our Mission

Core Outreach & Care Services aims to provide socially inclusive, personalised services for the Elderly and Adults with Disabilities so that they can regain or maintain their independence in the comfort of their homes.

We also support with social companionship, accompanied to the community services i.e. appointments, shopping, social activities. We promote Independence and social inclusion for people with learning disabilities or mental health by giving them the opportunities to access the community like everyone else.

Our Organisational Objectives

To provide individually focused and Outcome Based Services to help service users maximise their potential and independence in line with CQC Key lines of enquiries.

To work in partnership with the local authorities, Social Services, NHS, DOH, CQC and non statutory agencies to meet "Personalisation" objectives.

To enhance and utilise the skills and experience of staff in the
continuous development of the company and empower our staff towards a meaningful career progression.
